PRIOR TO APPLYING:
PLEASE READ THIS ENTIRE PAGE BEFORE BEGINNING THE APPLICATION PROCESS. BY CLICKING THE "APPLY" BUTTON TO THE LEFT, YOU AGREE THAT YOU HAVE READ AND ACCEPT ALL OF THE TERMS AS SET FORTH BELOW, INCLUDING THE PRIVACY POLICY OF KASSTING INC, PRODUCTION AND NETWORK, OUR TERMS OF USE, AND THE ELIGIBILITY REQUIREMENTS FOR THIS PROJECT.
QUESTIONNAIRE GUIDELINES:
1. Please do not use INTERNET EXPLORER when filling out the online questionnaire. This browser is not compatible with our system and your information will not be saved. We recommend using GOOGLE CHROME or FIREFOX.
2. When completing the online questionnaire, please do not use AUTO FILL or TAB through the questions. Due to the security measures we have put in place this could cause the system to think you are a robot and possibly lock you out.
3. EVERY QUESTION MUST BE ANSWERED. If a question does not apply to you, DO NOT skip over it - you must write "NA" in the space provided to acknowledge that you have read the question and it does not apply.
4. At the end of the online questionnaire process, you will be prompted to upload a few photos. Please have your photos saved to your desktop prior to starting the application process. We prefer if you provide us with at least 3-4 photos of yourself: At least one close-up, one full-body and a few fun shots. We prefer candid as opposed to professional photo's or "filtered" photos. These photos MUST LOOK LIKE YOU NOW! If you do not have photos readily available, or for some reason your photos do not upload properly, you can also email them to your casting producer to be uploaded later. Please avoid pictures with other people in them. We want to see you :)
5. You will be prompted to print your questionniare at the end. You are NOT required to do this. This is offered as a courtesy in case you would like to save a copy for yourself.
6. Although you will be prompted to upload a video at the end of the application process, this step is also not mandatory as we will be interviewing you in the near future. You can skip over this by selecting that option.
7. THE QUESTIONNIARE COULD TAKE UP TO 30 MINUTES TO COMPLETE. You must complete the questionnaire in one session. You will not be able to regain access after you submit. Please dedicate the allotted amount of time suggested to complete this in one sitting. If you do not complete the questionniare in time, you pause, walk away or leave the system unattended for too long, the system will time out after about 45 minutes and you will neeed to start over again.
8. Applicants MUST review and agree to the release, eligibility requirements, privacy policy and Terms of use at the end of the questionniare in order to submit their questionniare.
9. Any information provided in the application may be used for purposes relating to the show.
10. HAVE FUN! BE CREATIVE WITH YOUR ANSWERS! STAND OUT FROM THE CROWD :)
ELIGIBILITY:
1) You must be at least 21 years of age prior to commencement of production.
2) You must: (a) be a citizen of the U.S. and able to legally travel to and re-enter the U.S. from any foreign country in connection with your participation (if any) in the Program; OR (b) have valid U.S. Immigration status or a valid visa that allows you to stay legally in the U.S. to participate in the Program, as determined by Producer in its sole discretion, and be able to legally travel to and re-enter the U.S. from any foreign country in connection with your participation (if any) in the Program.
3) You are not currently, nor do you currently intend to be, a candidate for public office, and if there is any change in this representation prior to the initial exhibition of the Program in which you appear, you must immediately notify NBCUniversal (as defined below) at ReportingCandidacy@nbcuni.com. Becoming a candidate before or around the time of the initial exhibition of the Program may under certain circumstances have legal implications on the exhibitor's ability to exhibit the Program.
4) VelvetHM, LLC ("Producer") and NBCUniversal (as defined below) reserve the right to render ineligible any person who either of them determines, in their sole discretion, is sufficiently connected with the production, administration, judging, or distribution of the Program, such that his/her participation in the Program could create the appearance of impropriety. In making this decision, Producer and NBCUniversal may consider whether you are, or know or have known anyone who is, or has been within the last two (2) years, an employee, officer, director or agent of any of the following:
(a) Producer or any other person or entity involved in the development, production, distribution or other exploitation of the reality-based program currently entitled "Love To Love" (the "Program") or any variation thereof;
(b) NBCUniversal Media, LLC or any of its parent, subsidiary or affiliated entities, including, without limitation, its broadcast networks and studios, cable networks and studios, film businesses, digital businesses, parks and resorts (collectively, "NBCUniversal") (for more information about NBCUniversal businesses, please visit http://corporate.comcast.com or http://www.nbcuni.com);
(c) Any sponsor of the Program or its advertising agency; or
(d) Any person or entity supplying goods, services or prizes (as applicable) to the Program.
IMPORTANT: Please list any such relationships above when asked in the application; if none, write "None." If you become aware of any such relationships after completing this application, please immediately notify Producer in writing.
